How do I?

Register as a patient

Please visit reception and complete a registration form. You will need to provide some identification at the same time.

Alternatively, you can find an application form on line which should be completed and brought in with identification.

Make an appointment

Please telephone the practice at 8.30 for a morning appointment or 2.30 for an afternoon appointment. {Please be aware that patients are telephoned in the first instance by a clinician to establish the most appropriate person to see)

Repeat prescriptions

Please complete the on line request form available on our website. Alternatively, complete the request via the NHS app. Or call in to the practice to complete a request form. Please be aware we are receiving a high number of requests and we ask for 7 working days to fulfil your medication requests.

Obtain results

Please call the  surgery (01622 842212) after 3pm to enquire about your test results as our reception staff will have more time to deal with your request.

Note that the practice has a strict policy regarding confidentiality and data protection and we will only release test results to the person to whom they relate unless that person has given prior permission for the release of this data or they are not capable of understanding the results.

When you take your test you will be told how long it will be before the results are returned to the practice.

It is your responsibility to check your results and to make an appointment to discuss them with your doctor if you are advised to do so.
